Does anyone know if these are drops or ground spawns? If drops, what drops them?

It seems to be both, inside the mines in the Heroic Adventure?

The ground spawns in the Heroic Adventure all will be found in the mines. They will be either on top of a crate, on top of a loaded mine cart, or inside an empty mine cart.

The empty mine carts got me at first, since my normal halfling self is too short to see in them.
