Similar to Oubliette, you have to be in-zone in order to get the "Hero" portion of the achievement. The bonus/challenges will work for someone outside of the instance but not the Hero of Bloodfalls. Found out the hard way tonight while trying to get some folks completions.

Fian:
On the first wave of 3 mobs, "a shade beetle" can be mezzed.

The achievement "Underwhelming Shadows" is the opposite of what you would expect. To get it, don't kill "Pli Liako" quickly. Wait for him to cast a 5 min debuff, and then proceed with the mission. The debuff is pretty bad, caps health and mana at 75% plus I think it limits damage output as well. Regardless of whether you finish the mission with the debuff still active or not will not change anything. You still get the achievement if you win.

GoldenFrog:
If you kill the Golem before the Gargoyle, the Gargoyle will never unlock from 30% and can never be killed. But after a minute or two, the next wave starts and mission proceeds normally.
